If you are new or returning to Helena Vision Center, please download and complete the Medical History form to help speed up your visit. If you are unable to print this form, we will provide you with one when you check in for your appointment.
What to bring to your appointment:
We encourage all patients to review a copy of our Privacy Notice, which you are welcome to print out for your records. If you are unable to print out a copy, we’ll be happy to furnish one at your request during your next visit.
Helena Vision Center is credentialed to provide services for both vision and medical plans. The following list of insurance companies that we currently contract with is not necessarily complete and may be subject to change, so if you do not see your plan here, please call us at (406) 442-6814 for further information. You might also contact your insurance company to learn about in-network providers and your vision hardware benefits.
We bill contracted insurance companies directly on behalf of our patients. Any remaining balance for non-covered benefits and deductibles are the patient’s responsibility.
All insurance co-payments are to be paid at the time of service.
We understand that there may be times when financial difficulties arise. Under special circumstances temporary payment arrangements may be made. Patients whose accounts are on a temporary payment plan are required to make monthly payments to avoid collections and keep their accounts in good standing.
We accept cash, check, VISA, Mastercard, American Express and Discover. Don’t forget to take advantage of your tax-advantaged Flex Spending Account or Health Savings Account, which can be used to pay out-of-pocket costs of most products and services we provide.
